Description

ProMedica Toledo Hospital is one of the largest acute-care facilities in the region. Basic Purpose:

Receive and appropriately address all requests form the Care Navigation and care team members to ensure and promote safe and timely flow of patients between levels of health care and across post-acute settings.

Primary Duties

  • Initiate referrals with post-acute providers including SNF, LTAC, inpatient rehab, assisted living, group homes, home health, infusion, hospice, dialysis,  DME and oxygen, mental health and substance abuse treatment as identified by the care team.
  • Provide continued discharge planning updates, clinical updates and ongoing communication to post-acute providers as requested by the care team
  • Schedule primary care / specialty appointments for high risk patients as identified by the care team. Track the appointments throughout the stay and reschedule if discharge is delayed.
  • Schedule transportation as needed for a timely discharge and communicate transportation plans to the care team.
  • Develop and maintain collaborative working relationships with the care team and vendors.
  • Deliver the second required IMM (Important Message from Medicare) form to all Medicare patients who are inpatient status per CMS regulations.
  • Input accurate and timely data into established software programs.
  • Prioritize and utilize the chain of command for clinical and other situations that are beyond the scope of the CNRC in order to support flow and patient safety.
  • Provide data to support impact on service levels. Maintain ECIN statistics and close ECIN cases throughout the month as time allows, ensuring accurate end of month case closure.
  • Assess and prioritize workload to meet deadlines.
  • Performs other duties as assigned and maintain confidentiality.
  • Deliver IMM, 2nd IMM, MOON and Financial Responsibility forms in collaboration with facility Registration Teams and Utilization Management Teams.
  • Must demonstrate willingness and ability to adapt to change and support progressive movement and process changes.

  • 2-3 years of clerical experience working in a medical setting. 
  • History of positive rapport with patients, families, physicians, and interdisciplinary team

  • Must have general knowledge of medical terminology.

  • Must be able to prioritize tasks and complete work accurately in a fast paced environment.

  • Exceptional problem-solving, critical-thinking, organizational, interpersonal, and written/verbal communication skills

  • Ability to work in self-directed environment with attention to detail and follow-through.

  • Computer literate; Microsoft Office competency required. Must be able to navigate through the electronic medical record and document properly.

  • Must be willing to follow infection prevention protocol on use of personal protective equipment per policy.
